General Practice Policies

Appointments are reserved exclusively for you. We ask that cancellations or schedule changes be made at least 24 hours in advance to avoid cancellation fees.

Secure telehealth appointments are available throughout Connecticut. Clients are asked to participate from a private location that supports confidentiality and minimizes interruptions.

Your privacy is important. Information shared in therapy is confidential except in limited circumstances required by law, such as concerns about safety, abuse, or court orders.

A valid payment method is required for all clients. Fees, insurance benefits, and payment responsibilities will be reviewed prior to beginning treatment.

Threatening speech or behavior will not be tolerated and will lead to immediate discharge from the practice.
In the event a patient presents as a threat to self, others, or is gravely disabled and we are unable to make an adequate plan for safety:
Emergency contact will be contacted.
Crisis intervention or emergency services will be contacted to ensure patient safety.
Nourish The Heart Counseling does not provide after hours or weekend coverage. By signing this form you agree that if you are having a psychiatric or medical emergency you will not contact Nourish The Heart Counseling but rather seek emergent care by calling 911 or 211 for mobile crisis, or going to the nearest emergency department.
If you require a higher level of care Nourish The Heart Counseling will coordinate with those care providers and receive clearance from them prior to returning to outpatient level of care.

Because eating disorders can impact both physical and emotional health, collaboration with medical providers may be recommended. Medical monitoring or higher levels of care may be necessary when safety concerns arise.

If you are not, your therapist may ask you to put on additional clothing or they may cancel your appointment.
You will still be charged for this appointment.

Email and text messaging may be used for scheduling and administrative purposes. To protect your privacy, sensitive clinical information should be discussed during sessions rather than through electronic communication.
Med Management Practice Policies

Medication management is provided as part of a comprehensive treatment approach. Treatment recommendations are individualized and based on current clinical needs, safety considerations, and evidence-based prescribing practices.

Regular follow-up appointments are required to ensure medications remain safe and effective. Patients are expected to attend scheduled appointments and maintain ongoing treatment engagement. If you have not been seen in 3 months we will not refill any medicatons until you have been seen and if we do not hear hear from you after reaching out you may be discharged.

Medication refills are provided during scheduled appointments whenever possible. Requests for refills should be submitted several business days before medication is needed. Certain medications may require more frequent follow-up and cannot be automatically refilled.

Patient safety is our highest priority. Controlled medications, including stimulants and benzodiazepines, are prescribed cautiously and only when clinically appropriate. Additional monitoring, documentation, and prescribing requirements may apply. Anyone being prescribed a controlled substance will be required to get a urinalysis (UA) at intake or at the start of the prescription. You may be required to get further UAs at the prescriber’s discretion.

To promote safe and coordinated care, patients receiving psychiatric medications are expected to have a single psychiatric medication prescriber. Coordination with other medical providers may be required when necessary.
